Crashed 6 minutes after takeoff at Bradley's Farm, roughly 4 miles N of Louth Lincolnshire, near the Village of Little Grimsby. In the last moments of flight, 6 members of crew managed to parachute, but the parachute of Lloyd Douglas, RCAF failed to deploy and he was killed. The pilot, Lloyd Hannah, RCAF was killed when the Lancaster crashed into a farmer's field.
